Application privacy notice

HSS Recovery House, founded by CivicBridge NP, collects the details you enter into the enrollment application to review your housing request, discuss placement, and contact you.

What we collect

Your name, contact details, preferred state, move-in timeframe, deposit readiness, optional questions or contact instructions, and the date you consent. Authorized staff may add review notes and an application status. We do not request Social Security numbers, medical records, medication details, or criminal case records through this form.

How your information is used

Application data is saved in our website’s hosted database and is available through a password-protected staff portal. Authorized staff can review and export it for housing application follow-up. Please include only information needed for that purpose.
